We are sad to see you cancel your massage membership. Please complete the online form below to start the process of cancelling your monthly massage membership. Once we receive this completed form your membership will take 30 days to be completely cancelled. Any unused accrued massages remaining on your account are expire and are forfeited if not used within 30 days of the date we were notified of membership cancellation.
